Affiliate Instructions
Getting started with Epic Dental's Affiliate Program is super simple and sure to put a smile on your face. Just follow the three steps below and we'll have you up and running within one business day!
1. Complete Required Tax Forms
As an Affiliate Member of Epic Dental, we require a completed W-9 tax form before we can approve and activate your account. If you have not done so already, please download the W-9 form, and once completed, print and fax to 801-374-2253.
Until we receive your completed tax information, your account will not be active. A yellow message will appear on your Affiliate Account Dashboard, and any affiliate sales that are generated on your behalf will NOT be credited to your account.
2. Setting Up Your Affiliate Links
Adding your affiliate code to your links is quick and easy! Once we approve and activate your account, you just need to append your 5 digit Affiliate ID within any links sending traffic to EpicDental.com. That's it - you're on your way to earning!

To set up your affiliate links, simply create your own URLS and add ?affilaiteid=%AFFILIATEID% at the end, and we'll take care of the rest. It's really that simple!
3. Start Earning!
That's it! You're ready to start earning today! You may now use your Affiliate Account Dashboard to check your Affiliate Sales Activity, Recent Payouts, Update your Contact Information, and more!
