Affiliate Terms & Conditions

All affiliates sending traffic to EpicDental.com that leads to a sale will be given a percentage of that sale. The percentage of the sale that the affiliate receive is tiered, based on the overall amount of sales they produce:

• $0-200 sales/month = 15% Commission
• $201-400 sales/month = 20% Commission
• $401+ sales/month = 25% Commission

Affiliate commissions will be paid by check and will be written on the 5th of every month. The minimum payout for any affiliate is $50. Affiliate commissions do not apply to orders with coupons. All wholesale orders will receive a flat 5% commission. Terms and conditions are subject to change.

All affiliates are required to fill out a W-9 tax form before being approved. You can download it here. When you've filled out the form, print it out and fax it over to us: 801-374-2253.
